Q: Is the ground wire also electrical connected to metal surrounded the female jack so that the shield of a CAT6 shielded male connector is grounded?

Hi there, Not sure, but I had such bad ground surge issues from lightening that the surge was arcing from the outdoor CAT6 sheiled cable to the grounded RJ45 with this device before it could even ground the surge. The system here is 11 Poe cameras outdoors with underground Lan cables. Now I have bared the copper braided shield a few inches back from the RJ45 plug and soldered that directly into the building ground (as well as connecting the surge protector to ground). On the other end of the line outdoors I used Ubiquiti surge protectors that do not connect to ground (otherwise there would be a ground loop, i.e. a small current running through the shield all the time) but instead connect to the cable shield, also soldered in a few inches back from the RJ45 plug. This way there is only one earth in the building. Before doing all of this I popped several POE ports per lightening season, now it's been zero. So in answer to your question, I wouldn't rely on the earthing of the female jack at all, and am much happier with this method after the earthed female jack on its own was insufficient.

Q: Is this device POE+ (802.3at) compatible?

Yes, I'm using on a cat 5e link that powers an outdoor AP and protecting the switch port its connected to. Works great at both 24 and 48 v.

Q: how do you ground this? Can i attach it to a outlet ground?

Yes use the ground on the outlet.

Q: Can be installed with Cable Modem such as Cox modem? Because it says it is DSL.

This is just a in line surge suppressor for Ethernet going out from modem not incoming. This is for a cable if you have going outside to a computer or TV. This is just a basic lighting protection for your equipment if you have a line coming from your modem or router to equipment such as computer or game system.
